Information collected and used by ACDoctor
Service providers and persons who request services (collectively "Users") must provide contact information. At a minimum Users must provide their name, phone number, addresses or zip code, email address and specifics of the type of service(s) requested or offered. Service providers may additionally need to provide company credentials (e.g., trade licensing, insurance coverage, type of legal entity, operating hours).
Registration may be required for the service provider in order to sell a service using ACDoctor's web site. The service provider profile will be pre-populated with the information provided during the registration process. In addition, the service provider may also, if it wishes, provide additional information about their company through the use of a link to a third party web site or through communications to ACDoctor. A person requesting services may view the service provider profile or contact ACDoctor for information regarding the service provider.
In general, the above-described information is used by ACDoctor to match service providers and persons requesting services and to provide a more personalized experience to Users. A User's contact information may also be used by ACDoctor to contact the User to verify the status or to request more details of a service request.
ACDoctor encourages Users to submit comments or evaluations of their experiences with ACDoctor.com and with third party service providers located through our web site or provided by ACDoctor. Please be advised that any comments or feedback that you provide or send to ACDoctor may (and you authorize us to), in our reasonable discretion, be reproduced, copied, distributed, displayed and, modified for formatting purposes.

Log Files

By preserving our right to make disclosures in such instances ACDoctor is not undertaking an obligation to defend you or others from injury or to seek relief for those injuries.
Our policy only addresses the use and disclosure of information we collect, different rules regarding collection or dissemination may apply to information that you disclose to other parties, whether a buyer or seller on our system or other sites on the Internet. Surveys and Contests
ACDoctor may request information from users via surveys or contests. Participation in these surveys or contests is completely voluntary and the user therefore has a choice whether or not to disclose this information. Contact information will be used to notify winners and award prizes. Survey information on the site will be used for purposes of monitoring or improving the use and satisfaction of the site. Choice/Opt-Out
Subsequent to registration, service providers may at any time decline to receive future e-mails by editing their profile ("opt-out" election) or selecting the unsubscribe option under Join our mailing list. Of course, this opt-out election excludes e-mails that are part of the process of requesting a service.
Editing/Updating user information
Users can correct or change their contact information or service professional profile through the following options:
• Online (registered service professionals ONLY): A registered service professional consumer may edit their ACDoctor account information at any time by using their supplied user name and password.
• E-mail: Users can change their information by sending an e-mail to info@acdoctor.com .
• Phone: User information can also be modified by calling customer service at (866) 264-1479
Security Precautions
ACDoctor is aware of Internet users' concerns about the privacy of their information and has taken several security precautions to protect the integrity of this information. User information is stored on secure servers guarded by both a firewall as well as system- and database-level security. In addition, your service provider ACDoctor account information is password-protected to restrict access to this information. You may edit your ACDoctor account information by logging into the account with your user name and password, which may be done automatically through a cookie. ACDoctor will never ask you for your password in an unsolicited phone call or in an unsolicited e-mail and we recommend that you do not divulge your user name and password information to anyone that you do not wish to have access to your account. We will consider anyone that accesses our site with your user name and password to have your full authority and approval to conduct business on your behalf, to bind you to legally enforceable agreements, and to access your account, including any information that you may consider private. To help you maintain the security of your information, ACDoctor will disable your user name and password for any reason at your request, provided that you first supply us with the proper information that identifies you as the user. Only those at ACDoctor who require access to user data are granted access to this information. Access to user data is further limited because it is available only through a dedicated administrative interface and through use of a restricted user ID and password. In addition, any ACDoctor personnel (whether a ACDoctor employee or, in limited instances, a consultant or independent contractor) with access to data that can identify users individually is required to sign ACDoctor's standard Nondisclosure and Development Agreement, which requires user data be kept confidential.
Unfortunately, no data transmission over the Internet can be guaranteed 100 percent secure. As a result, while we strive to protect your personal information, ACDoctor cannot ensure or warrant the security of any information received from our online services or other information you transmit to us. Once we receive your transmission, we will use commercially reasonable efforts to ensure its security on our systems. ACDoctor's internal web se3curity policies are reviewed and revised on a regular basis.
Notification of changes
ACDoctor will occasionally update this privacy policy. If there are changes to this privacy policy or in how ACDoctor will use your personally identifiable information, ACDoctor will notify users by way of an email or Users preferred method of contact. Users will have a choice as to whether or not we use their information in this different manner.
